Core Clinical Services

This is proactive, skilled nursing focused on prevention – not reactive care.

When appropriate, this plan can be layered with:

  • Care+ Kidney: Early kidney health monitoring, diet education and fluid guidance
  • Care+ Lungs: Lung-strengthening exercises and oxygen optimization

Our nurses document ongoing skilled needs consistent with the Department of Labor’s requirements so that we:

  • Support ongoing authorization of skilled hours
  • Support adding new conditions to your White Card
